Please have this done as early in advance as possible but not less than one week prior to the start of practice. CLICK HERE TO GO TO DOWNLOADABLE ATHLETIC FORMSRequirements for Playing in an Interscholastic Contest- Students will not be issued a game uniform nor will they be allowed to participate in an interscholastic contest until the following items have been completed: ▪A copy of the student's birth certificate is on file ▪The student satisfies all rules of academic eligibility - See Athletic Code - Item #5.▪If a transfer student, Ms. Vargas or Mrs. Schmaltz is contacted before trying out for any sport. ▪A fully-signed copy of the athletic code is on file with the office.▪All sports participation fees have been paid, $150.00 /sport. See Ms. Vargas if you have any questions or concerns regarding the athletic fees. Transfer Eligibility- Any student planning to play sports and who has transferred to St. Mary's from another high school from August 1, 2010 to the present must see Ms. Vargas (Athletic Director) or Mrs. Schmaltz (Secretary) to receive eligibility information or clearance. Students transferring schools are not eligible to participate in sports unless procedures and/or forms have been completed. Not following these procedures would result in penalties for the school, the athletic program and your child (forfeitures, sanctions, ineligibility, etc.).Academic Eligibility- Eligibility is determined four times each school year: first quarter grades, first semester grades, third quarter and second semester grades. Detailed information regarding academic eligibility is available in the school office. Athletic Fee Receipt- There is a fee of $150.00 per sport due prior to competition, beginning in each athletic season. Please plan and prepare to pay this fee at least 7-10 days prior to the first game. Any students whose fee has not been paid will not be eligible to participate in a game/match. Do not wait until the week of the first game as we may not be able to accommodate issuing a uniform. Check with the business office about multi-student discounts for children of the same family playing sports during the same season (Fall, Winter or Spring). Important Dates and Deadlines- ▪Fall Sports: Boys' & Girls' Swim begins August 1, 2011. Football, Volleyball, Boys' & Girls' Cross Country, & Boys' and Girls' Golf will begin practice on Monday, August 8, 2011. Physical/Permission, transportation forms and Family & Coach Agreement are due no later than 7/25/11. For athletic information please call 602-251-2502. All other forms and the athletic fee ($150.00) receipt are due in the athletic office before a game jersey will be issued or 1 week prior to the first athletic contest, whichever occurs first. ▪Winter Sports: Boys' & Girls' Basketball & Soccer begin practice on October 31, 2011. Physical/Permission, Transportation and Family Coach Agreement forms are due 10/14/2011. All other forms and athletic fee receipt ($150.00) are due one week prior to the first athletic contest or before a game jersey is issued, whichever occurs first.▪Spring Sports: Boys' & Girls' Tennis will begin 1/2/2012. Baseball, Softball, Boys' & Girls' Track will begin Feb. 6, 2012. Boys' Volleyball begins practice on February 13, 2012. Physical/Permission, Transportation & Family and Coach Agreement forms due 1/13/2012. All other forms and athletic fee receipt ($150.00) are due one week prior to the first athletic contest or before game jerseys are issued, whichever occurs first. Athletic Equipment- Uniforms and equipment provided by the school must be returned at the conclusion of the season. Items not returned will be charged to student's account. Athletic Awards- At the conclusion of each sport's season, the head coach will arrange for an awards banquet for that sport. Qualifications for Freshman and Junior Varsity Athletic Awards- All players who have remained with the team throughout the season will receive a Saint Mary's (gold foil) certificate. Any player who has resigned or has been removed from the team due to academic or disciplinary reasons will not qualify for post-season awards. Qualifications for Varsity Letter- Any varsity athlete who participates in more than half of the contests for that sport's season may at the coaches' discretion earn a varsity letter provided that player has not resigned or been removed from the team for academic or disciplinary reasons. A varsity coach may at his or her discretion award a letter to an athlete who didn't qualify under the contest participation criteria if the coach determines that the athlete's contribution to the team warrants receiving the award. Varsity athletes who previously received a letter for any sport will be awarded a certificate for subsequent varsity awards in that sport.
